Breast Lift in the San Francisco Bay Area

Norcal plastic surgery service breast lift 1

Pregnancy, breastfeeding, weight change, aging, and gravity can alter the position and shape of the breasts. Volume settles lower, skin stretches, and the nipples may no longer face forward. A breast lift restores structure by reshaping and elevating the existing tissue.

At NorCal Plastic Surgery, board-certified plastic surgeon Dr. Timothy Rankin plans breast lift surgery around the patient’s anatomy, desired size, skin quality, and long-term priorities. The objective is not simply a higher breast. It is a more balanced relationship among breast shape, nipple position, and the body as a whole.

What Is a Breast Lift?

A breast lift, or mastopexy, reshapes and elevates breasts affected by sagging, medically called ptosis. Excess skin is removed, the breast tissue is reorganized, and the nipple and areola are repositioned when needed.

A lift changes breast position and contour but does not inherently add volume. Patients who want more fullness may combine it with augmentation, while those who want smaller, lighter breasts may need reduction.

What Can a Breast Lift Improve?

Breast lift surgery may address:

  • Breasts that sit lower than desired
  • Nipples that point downward or rest below the breast fold
  • Stretched or enlarged areolas
  • Loose skin after pregnancy or weight loss
  • Flattened or elongated breast shape
  • Differences in breast position or contour
  • Loss of upper-breast shape associated with tissue descent

A lift cannot create perfect symmetry or stop future aging. Pregnancy, weight fluctuation, skin quality, and gravity will continue to influence the breasts after surgery.

Breast Lift, Augmentation, or Reduction?

These procedures solve different concerns.

A breast lift restores position and shape using the existing tissue. Breast augmentation adds volume with implants or fat. Breast reduction removes breast tissue and skin to create a smaller, lighter breast.

Patients satisfied with their volume in a supportive bra may need a lift alone. Those who want more upper-breast fullness may consider a lift with an implant or fat transfer. Patients troubled by excessive weight or physical discomfort may be better candidates for reduction.

The correct plan depends on breast position, desired size, tissue quality, and the amount of upper-breast fullness the patient wants.

Do I Need a Breast Lift With Implants?

An implant restores volume but cannot reliably correct significant sagging. When the nipple rests below the breast fold or the skin envelope has stretched considerably, adding an implant alone may leave the breast fuller but still low.

Combining a lift with augmentation allows Dr. Rankin to address volume and position in the same plan. The implant must remain appropriate for the tissues. Using excessive volume to avoid a needed lift can increase weight on the skin and compromise long-term shape.

Not every patient needs an implant. Existing breast tissue may provide enough volume once it is reshaped and elevated.

Who Is a Candidate for Breast Lift Surgery?

A breast lift may be appropriate for adults with sagging or stretched breast tissue who are in suitable health for surgery. Candidates should maintain a relatively stable weight, avoid nicotine, and have realistic expectations about scars and future breast changes.

Dr. Rankin evaluates:

  • Breast and nipple position
  • Degree of skin laxity
  • Existing breast volume
  • Areola size and position
  • Breast symmetry
  • Skin and tissue quality
  • Previous breast surgery
  • Pregnancy and breastfeeding history
  • Future pregnancy and weight-change plans
  • Breast-health history

Patients planning pregnancy or substantial weight loss may wish to postpone surgery because either can alter the result.

Which Breast Lift Incision Will I Need?

The incision pattern depends on the degree of sagging, skin excess, breast size, tissue quality, and the amount of reshaping required.

Periareolar Lift

A circular incision is placed around the areola. This approach may suit carefully selected patients requiring limited correction. It cannot reliably address more significant tissue descent or excess skin.

Vertical Lift

The incision extends around the areola and vertically to the breast fold, creating a lollipop pattern. It allows greater reshaping and skin removal than a periareolar lift.

Anchor Lift

The incision follows the areola, extends vertically to the breast fold, and continues along the fold. Also called an inverted-T or Wise-pattern lift, it provides the greatest access for reshaping breasts with substantial skin excess or descent.

More extensive correction requires longer incisions. The shortest scar is not always the best choice if it cannot create or support the necessary shape.

How Is a Breast Lift Performed?

Breast lift surgery is performed on an outpatient basis. Dr. Rankin removes excess skin, reshapes the breast tissue, and repositions the nipple and areola to a more appropriate level. Enlarged areolas may be reduced when indicated.

The nipple usually remains attached to underlying tissue so that blood supply and nerve connections can be preserved, although sensation and breastfeeding ability may still change.

The current NorCal Plastic Surgery page states that long-acting local anesthetic is used during breast surgery to support postoperative comfort. The complete anesthesia and pain-management plan is individualized and discussed before surgery.

What Is Recovery Like?

Swelling, bruising, tightness, tenderness, and temporary changes in breast or nipple sensation may occur. The breasts can initially appear high, firm, or slightly uneven before the tissues settle.

Some patients return to nonstrenuous work within several days, but recovery varies with the extent of lifting, combined procedures, job demands, and individual healing. Walking is encouraged early, while lifting and upper-body exercise remain restricted.

Activity is reintroduced gradually. Patients should avoid pressure on the breasts and delay strenuous exercise until Dr. Rankin confirms that healing is sufficiently advanced. Those planning surgery before a wedding, vacation, or other important event should allow time for swelling to improve, scars to begin maturing, and clothing fit to stabilize.

What Will Breast Lift Scars Look Like?

Every breast lift creates permanent scars. Their location follows the incision pattern required for the correction.

Scars are usually most visible early in recovery and may appear raised, firm, pink, or darker than the surrounding skin. They typically soften and fade over time, but they do not disappear completely. Genetics, skin tone, incision tension, nicotine exposure, sun protection, and postoperative care influence scar maturation.

Choosing an incision based only on scar length can compromise breast shape. Dr. Rankin balances the necessary correction with the least extensive pattern capable of supporting it.

When Will Results Be Visible?

The breasts appear higher immediately, although swelling and firmness obscure the final shape. Contours become more natural as the tissues soften and settle over the following weeks and months.

Scars continue to mature beyond the initial recovery period. Minor asymmetries may remain because no two breasts begin or heal identically.

Results can be long lasting, but they are not permanent in an unchanging sense. Pregnancy, aging, gravity, and weight fluctuation may alter breast shape over time.

Breastfeeding and Breast Health After a Lift

Some patients can breastfeed after a breast lift, but future milk production cannot be guaranteed. Surgical technique, the extent of reshaping, previous pregnancy, and individual anatomy may affect breastfeeding ability.

Patients should continue age- and risk-appropriate breast cancer screening and inform the imaging center about previous breast surgery. Based on age, symptoms, history, or established screening guidance, breast imaging may be recommended before or after surgery.

Risks and Limitations

Potential risks include bleeding, infection, scarring, asymmetry, delayed healing, fluid collection, contour irregularity, changes in sensation, partial or complete loss of nipple or areola tissue, difficulty breastfeeding, fat necrosis, recurrent sagging, and revision surgery.

Combining a lift with an implant introduces implant-related risks, including rupture, capsular contracture, malposition, and future implant surgery.

A breast lift cannot guarantee perfect symmetry, scarless surgery, permanent upper-breast fullness, or a result unaffected by future pregnancy and aging. Dr. Rankin will review the expected improvement, alternatives, and individual risks before treatment.

Shape Before Size

A breast lift is a structural procedure. Its success depends on how effectively the tissue is reshaped and supported, not simply how high the nipple is placed or how much skin is removed.

Dr. Rankin considers breast width, volume, skin quality, nipple position, and the relationship between the breasts and torso before recommending a lift alone, augmentation, reduction, or a combined procedure.

Frequently Asked Questions About Breast Lift Surgery

A lift may be appropriate when the breast tissue has descended, the nipple points downward, or the nipple rests below the breast fold. A consultation is necessary because volume loss, skin laxity, nipple position, and breast size all influence whether a lift, augmentation, reduction, or combination is most appropriate.

A lift can reshape and elevate existing tissue, but it may not create the degree of upper-breast fullness some patients want. An implant or fat transfer may be considered when additional volume is needed. The recommendation depends on existing tissue, skin quality, and the desired contour.

Yes. A breast lift can be performed using existing breast tissue when the patient is comfortable with her current volume. An implant is considered only when additional size or fullness is desired or needed to achieve the planned shape.

Results may remain attractive for many years, but the breasts continue to age. Pregnancy, weight change, gravity, skin elasticity, and breast size influence longevity. Maintaining a stable weight and avoiding nicotine support the tissues, but no surgery can permanently prevent future sagging.

Temporary changes in nipple or breast sensation are common. Sensation often improves as swelling subsides and nerves recover, but some changes may be prolonged or permanent. The likelihood depends on anatomy, surgical extent, healing, and previous breast procedures.

Breastfeeding may remain possible, but it cannot be guaranteed. Surgery can affect milk ducts, glandular tissue, nerves, or nipple sensation. Patients who hope to breastfeed should discuss this priority with Dr. Rankin before choosing a procedure or timing surgery.

It may be. A mommy makeover is individualized and may include a breast lift, augmentation, reduction, or another breast procedure alongside abdominal or body contouring. Only procedures appropriate for the patient’s anatomy and goals are included.

Cost varies with the incision pattern, surgical complexity, anesthesia, facility requirements, and whether an implant or another procedure is included.
